Why do most OKR implementations fail?

The common causes are key results written as task lists, too many objectives at once, no weekly check-in rhythm, OKRs linked to individual bonuses, and leadership that sets OKRs then stops referring to them. Each is a process failure, not a problem with OKRs as a method.
